NACAC Conference Registration Information

Registration Forms

To register for the conference, download a PDF version of the registration form, print it, complete it, and return it to NACAC. (To read and print a PDF file, you must have Adobe Acrobat Reader, which is available free at http://www.adobe.com/products/acrobat/.)

If you are registering for the child care or the teen program, download and print the PDF versions of these forms (childcare formteen program form) and send them to NACAC.

If you are unable to download the PDF files, please send an e-mail to info@nacac.org to request the printed registration booklet. Be sure to include your name and mailing address with your request for the booklet.

Registration Instructions

  • Complete all portions of the form on the next page. Please print clearly.
  • Early registrant discounts are available only for registrations postmarked by June 30. After June 30, full conference fees increase by $55.
  • Please calculate the total amount due on the registration form. All registrations must be accompanied by payment (check, money order, credit card, or organizational purchase order). Make checks and money orders payable to NACAC. Canadian credit card payments will show up on your statement as paid to CHOICES Adoption & Counselling Services.
  • All fees are the same in U.S. or Canadian dollars, with the exception of membership fees. Canadian membership fees are higher to reflect significantly higher postage costs. 
  • Parent couple registration rates are designed for two people who are raising a child or children together.
  • The membership discount is available to all current individual members, one representative of each parent group member, up to 20 employees of organizational members, up to 20 parents affiliated with enhanced parent group member, and all representatives of employees of national/corporate members.   • To attend any of the pre-conference sessions, please complete the Pre-Conference Sessions section on the registration form. Parent couples should indicate which person is attending what session. Parents can attend the same sessions or different sessions, or just one parent can pick a session.
  • Saturday’s award luncheon is included in full conference registration and the Saturday-only registration fees. If you need additional tickets for guests, please contact NACAC.
  • Please contact NACAC to verify your conference registration if you do not receive a written confirmation letter within two weeks.
  • By indicating institute and workshop preferences on the registration form, you help us to more accurately assign rooms based on expected attendance. However, you are not bound to your choices at the conference; simply get to sessions early enough to get a seat.
  • If you need special accommodations to enable you to more fully participate in the conference (such as sign language interpretation), please contact NACAC.
  • If you prefer NOT to be included on a registrant list, please check the box on the registration form under the Registrant Information section.
  • For written conference cancellations received by June 30, we will refund fees paid less a service change of $50. For cancellations of only childcare or teen programming or the pre-conference session (but you still attend the conference), the service charge is $25. We cannot refund any fees when cancellation notice is not received in writing by June 30.
  • If your payment is for multiple registrations, please send all of the registration forms together.
