AdoptWalk Team
Registration Form

If you are sponsoring a team of walkers for AdoptWalk, please complete the form below. Please note that each individual on a team must complete and submit an Individual Registration Form (or download PDF) or a Family Registration Form. All walkers should retain the Sponsor Pledge Form to collect their pledges for turn-in at the walk site to their team leader.

Team leaders must turn in all members' forms to earn 25 percent of proceeds.

I assume all risks associated with participating in this event. Having read this waiver and knowing these facts and in consideration of your accepting my entry, I, for myself and anyone entitled to act on my behalf, waive and release the North American Council on Adoptable Children, its board and staff, and all sponsors of the event, their representatives, and successors from all claims or liabilities of any kind arising out of my participation in this event.
